Attendant Care Indiana

Who Is Eligible?

Indiana Medicaid Family Caregiver
SFC Agency Indiana

1. Live in Indiana

You must live in the State of Indiana and need long term care assistance with 2 ADLs (Activities of Daily Living). Hoosier Caregivers must be 18 years or older.

2. Live Near or With Your Caregiver

For the Structured Family Caregiving Indiana Program, you must live in the same home as the Caregiver. For the Attendant Care Program, your caregiver just needs to live nearby.

3. Have Medicaid and Get On a Waiver

Qualify for Medicaid or already have Medicaid and meet the financial and asset requirements for the Medicaid Aged & Disabled Waiver. (The Paid.care team helps here)
